The first “standard” document for hospital standardization was implemented by the American College of Surgeons Board of Regents on the 20th of December in 1919. The contents of the document were compiled based on years of visits to hospitals, questionnaires and research about hospital record management and general hospital administration at the time. There was a dire need for this document. In 1910 Ernest A. Codman from Boston shone a light on the appalling state of hospital records. He spoke on the related difficulties with the safety and wellbeing of patients. He went on to make his formal statement at the Clinical Congress of Surgeons of North America in 1912, informing those present that adequate records would be vital in order to study the conclusions found by the hospital, as well as the overall state of the efficiency of the hospital, so that improvements could be made.
