identify threats or abuse of the person it will be against and obtain evidence
consult with an attorney on which steps to take depending on the degree of the offenses
go to the courthouse or website and obtain the proper forms and fill them out
once submitted you'll receive a court hearing which you will attend on the day they've assigned you at the time on the docket
serve the order to the abuser
if granted keep copies of all documents and the judge will explain the rules and regulations
each state is different but that's the general procedure
