The nurse should ask about the intensity and frequency of chest pain, ask the patient to describe the pain as best he can, if it radiates to the other side of the chest, likewise he or she should ask about dyspnea, ask him to describes from a scale of 1 to 10 the lack of oxygen he presents, the frequency and the moments in which it is presented, all this to notify the health care provider so he sends the pacient to perform stress tests, pulmonary function tests and chest ultrasound.

When a nurse is obtaining health history from a client having a dyspnea and chest pain, she asks some question to diagnose correctly:

She mainly asked how frequent is chest pain is occurring and she asked the frequency of the chest pain. Also, she asked about the chronic and acute nature of dyspnea.
